If you have trouble competing the online form, a printable version is available here HMS Print Registration Form. Please complete the form on your computer and submit via email to forms@hyperbaricmedicalsolutions.com.

If you prefer to print and complete the form by hand, you may fax it to 516.802.5026, mail it to your local HMS location, or simply bring the completed form to your consultation (please arrive 30 minutes ahead of your scheduled appointment time).

Accepted Insurances

As an independent, free-standing provider of hyperbaric oxygen therapy, HMS is a member of most major insurance plans/networks where it operates. The insurance plans accepted will vary by state. As a non-hospital provider, patients’ financial responsibility is far less than in a hospital, which charge multiple co-pays and have higher co-insurance responsibilities. We are proud to be able to provide more affordable access to this valuable service.
